There are many forces that impact health delivery systems. Choose one aspect that influences how health delivery systems are challenged, and describe the issue. Examples include, but are not limited to, access to care, quality health care improvement, lack of coordinated care, continuity of care (medical home), etc.

After reviewing the Logic Model Development Guide from the readings, create a logic model for a new program you would like to fund to address this impact on the health delivery system. Discuss how the program can be applied to reduce the impact on the health delivery system.

For most people, health means simply the absence of disease- an all or
non phenomenon, but this is not true in real life. The oxford dictionary defined
health as” The soundness of the body”: that condition in which the body succeeds
in adaptation to its environment and functions efficiently.

adaptation implies that health is a changing process or state. Early in the 1950s,
The World Health Organization (WHO) defined health as a state of complete
physical, mental and social well being and not merely the absence of disease or
infirmity.

The problem in this definition is related to the meaning of the word
“complete” and who decides this completeness. Although the WHO definition of
health is “theoretical”, it implies a positive valuation of health and encourages
almost all world nations to work for the improvement of their people health.
